Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Indiana PTA state license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross required or may be required.
• Experience working in a skilled nursing facility, long-term care, short-term rehabilitation, or assisted living preferred.
• Ability to manage caseloads in a small SNF environment with approximately 34 certified beds preferred.
• Experience collaborating with licensed Physical Therapists and interdisciplinary teams preferred.

Assignment Details

• Facility Type: Skilled Nursing Facility (SNF) with long-term care, short-term rehab, and some assisted living.
• Shift Name: Days.
• Schedule: Primarily Monday through Friday; schedule flexible with 4 or 5 days per week options and occasional weekend coverage possible.
• Guaranteed Hours: 35 hours per week guaranteed.
• Start Window: Beginning of May start date, flexible.
• Duration: Assignment length is flexible.
