Description
Make a meaningful impact as a Licensed Clinical Social Worker in Prescott, AZ. This hospital-based position requires an experienced LCSW or LMSW who will advocate for patients across inpatient units and the emergency department, complete social work and mental health assessments, support discharge planning, and connect patients with community resources addressing homelessness, substance use, and social determinants of health.
Minimum Qualifications
- Master’s degree in social work (MSW) from an accredited educational program.
- Current state license in good standing. LMSW or LCSW required; indicate level on application.
- Two years of supervised clinical experience providing direct patient care, including behavioral health assessments and discharge planning in acute care settings.
- Experience working in inpatient and emergency department environments. Must be comfortable covering ED assessments and inpatient consults.
- Proficiency with Cerner electronic medical record system.
- Experience completing Title 36 petitions, reporting to DCS and APS as needed, and managing caseloads averaging 12–16 consults per day.
- Ability to complete required department onboarding and orientation (approximately 12 hours formal orientation; 1–2 week onboarding with Social Work Manager).
- Must meet facility radius rule: candidates may not be currently employed full time, part time, or PRN with any CommonSpirit-associated facility (including CHI or Dignity) within the past year.
- Preferred: Prior experience with maternal-child assessments, ICU consults, and family birthing center social work.
Assignment Details
- Facility type: Hospital.
- Shift: Days.
- Schedule: Five 8-hour shifts, typical workday 0800–1600 (typical day may be 0800–1630 with occasional flexibility required).
- Workweek: 40 hours per week.
- Coverage areas: Inpatient units, Emergency Department, Family Birthing Center, ICU.
- Orientation: Department-specific onboarding plus approximately 12 formal orientation hours.
- Weekend frequency: Rare; approximately every 3–4 weeks if needed.
- Float requirement: May be required to cover emergency department shifts and other hospital units as needed.
