Minimum Requirements
- Doctoral degree in Physical Therapy (required by client).
- Active Oregon Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home Health experience preferred.
- Experience with HomeCare Homebase EMR preferred.
- Must be able to perform PT visits including SOC, PT evaluations, recertifications, ROC, and supervisory visits.
- Ability to cover Jackson County service area and travel between patient homes. Mileage reimbursed between visits from first patient to last patient.

- Shift Details: Mon-Fri, 0800-1630.
- Weekly Hours: 40 hours per week guarantee.
- Productivity expectation: 30 points.
