Minimum Requirements
- Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
- Current, unrestricted West Virginia physical therapy license required prior to start.
- BLS certification from AHA or ARC may be required.
- Experience with OASIS visits and add-on evaluations required.
- Home health experience preferred.
- Ability to complete documentation in a timely manner and meet productivity expectations (points-based productivity may apply).
Assignment Details
- Facility type: Home Health.
- Shift: Days.
- Shift hours: Monday–Friday, 08:00–16:00.
- Weekly guaranteed hours: 40.
- Coverage area: Kanawha County; local travel between patient visits required. Mileage reimbursement between first and last patient is provided.
- On-call/weekend requirement: None.
- Dress code: Royal blue scrubs.
- Must support community- and state-focused patient populations.
