Minimum Requirements
- Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
- Current, unrestricted WV state license or ability to obtain state licensure prior to start.
- BLS certification from AHA or ARC may be required.
- Experience with OASIS visits and home health documentation preferred.
- Home health experience preferred.
- Ability to perform add-on evaluations as required by the client.

- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Monday–Friday, 08:00–16:00.
- Weekly Hours: 40 (guarantee noted by client).
- Coverage Area: Kanawha County.
- Dress Code: Royal Blue scrubs.
- Mileage reimbursement between visits from first patient to last patient: Yes.
