Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Colorado physical therapy license is required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing outpatient physical therapy, including comprehensive evaluations and development of individualized treatment plans.
• Ability to perform 60-minute evaluations plus treatment sessions.
• Competence with documentation, goal writing, letters of medical necessity, and charge entry in compliance with state and federal guidelines.
• Ability to supervise and provide direction to support staff and students when applicable.
• Maintain professional licensure and complete state-mandated continuing education requirements.
• Preferred: prior experience in a clinic using electronic medical records and experience with functional outcome measures.

• Facility Type: Outpatient.
• Shift Pattern: 5x8.
• Duration: 13 Week assignment.
• Caseload: Outpatient PT with 60 minute evaluation and treatment sessions.
