Minimum Requirements
- Doctoral, Master's, or Bachelor's Degree in Physical Therapy from an accredited program.
- Active Florida Physical Therapy license required prior to start.
- Proficiency with HCHB and OASIS required.
- BLS certification from AHA or ARC may be required.
- Experience delivering in‑home physical therapy, including comprehensive evaluations, home safety assessments, and development of individualized Plans of Care.
- Ability to manage an independent caseload and meet productivity expectations (client expects approximately 40 hours weekly and productivity target of 25 points).
- Experience coordinating care with interdisciplinary teams and adhering to Medicare/VA/payer documentation standards.
- Valid driver’s license and ability to travel between patient homes. Mileage between visits is reimbursed by the client.
- Preferred: Immediate availability.

- Coverage area: Orange County, primarily east and central Orlando.
- Weekly hours: 40 (client weekly guarantee).

Job Responsibilities
- Conduct comprehensive in‑home physical therapy evaluations assessing mobility, strength, balance, pain, endurance, and safety.
- Develop, implement, and update individualized Plans of Care aligned with physician orders and patient goals.
- Deliver skilled interventions to improve mobility, gait, balance, strength, endurance, and pain management.
- Provide patient and caregiver education on diagnosis, fall prevention, home exercise programs, and durable medical equipment use.
- Perform home safety assessments and recommend environmental modifications to reduce fall risk.
- Maintain accurate, timely documentation in the electronic medical record, including evaluations, progress notes, and discharge summaries.
- Communicate clinical changes or barriers promptly to ordering physicians and agency leadership.
- Ensure adherence to Medicare, VA, and payer requirements regarding skilled need and homebound status.
