Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active New Jersey Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience required. Experience conducting start of care, evaluations, revisits, reevaluations, and discharges in a home health setting is required.
• Experience with HCHB (HomeCare HomeBase) and PointClickCare / PointCare workflows required.
• Must be able to travel within local coverage areas including Newark, Nutley, Belleville, and Bloomfield as part of visits.
• Minimum education requirement: Bachelor's degree.
• Preferred: prior home care-focused physical therapy experience.

Assignment Details

• Facility type: Home Health.
• Shift name: Days.
• Shift details: Days 08:00–16:00 (8 am–4:30 pm noted in job description).
• Weekly hour guideline: 30 hours per week.
• Productivity expectation: 30 points.
• Weekend requirement: weekends optional. No on-call requirement.
• Responsibilities: start of care, patient evaluation, revisits, reevaluation, and discharge.
• Dress code: navy blue scrubs.
• Mileage reimbursement: travel between first and last patient visits is reimbursed as specified by the client.
