Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapy license required prior to start of assignment.
• Current CPR certification required.
• Valid driver’s license, current vehicle insurance, and reliable personal transportation for travel within the coverage area.
• Experience in home health settings preferred. Experience serving adult and geriatric patient populations preferred.
• Ability to perform patient evaluations, develop and implement plans of care, and document progress per agency and regulatory standards.

• Coverage area includes Butler, Warren, Clermont, and Hamilton counties with a special focus on Warren and Clermont counties.
• Position status: Full time.
• May require travel within the designated county coverage area during work hours.

Impacting Patient Care

As a Physical Therapist in home health you will evaluate functional status, develop and implement therapeutic interventions, and educate patients and caregivers. You will document progress, collaborate with physicians and interdisciplinary team members, and report changes in patient condition per agency protocol.
