Minimum Requirements

• Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Washington State Physical Therapist license required to start the assignment.
• Minimum 1 year of home health physical therapy experience required.
• BLS certification from AHA or ARC may be required.
• Comfort with patient-to-patient travel during the workday; mileage reimbursed between patient visits (client policy).
• Willingness to follow client dress code: navy blue top and scrubs with khaki shorts, per site policy.
• Preferred: additional home health experience beyond the minimum.

• Facility Type: Home Health.
• Shift Name: Days.
• Schedule: 5x8 shifts, days (start and end times provided by facility).
• Weekly Hour Guarantee: 32 hours.
• Productivity Expectation: 30 points.
• Coverage Areas: Centralia and surrounding areas; candidate should be able to travel between patient homes within the coverage area.
• On-call/Weekend Requirement: No weekend or on-call requirement.
