Minimum Requirements
- Doctoral Degree in Physical Therapy from an accredited program. (Client requires Doctorate Physical Therapy)
- Active Oregon Physical Therapist license required to start the assignment.
- BLS Certification from AHA or ARC may be required.
- Home Health experience preferred. (Client preference)
- Experience with HomeCare Homebase EMR preferred. (Client preference)
- Ability to perform home health PT visits including SOC, PT evaluation, recertification, ROC, and supervisory visits.
- Willingness to travel within and provide coverage across Josephine County service area.
Assignment Details
- Facility Type: Home Health
- Shift Name: Days
- Shift Details: Monday–Friday, 0800–1600 (client lists Mon–Fri 0800–1630)
- Weekly Hour Guarantee: 40 hours
- Productivity Expectations: 30 points (per client guidance)
- Mileage reimbursed between visits from first patient to last patient.
- May be required to provide coverage across the county service area; candidate should be able to travel between patient homes.
