Minimum Requirements
- Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Preferred: Bachelor’s or higher listed in original description.
- Current Physical Therapy licensure in the state of Washington required to start the assignment.
- Current CPR certification required.
- Current driver’s license and vehicle insurance. Access to a dependable vehicle or public transportation required.
- Ability to function in home health environments and provide care in diverse home situations.
- Experience with home health visits, including completion of patient evaluations, plan of care development, progress notes, and documentation per agency policy preferred.
- Familiarity with outpatient rehabilitation principles and goals-oriented treatment planning preferred.

- Documentation and productivity expectations: Complete evaluations and plan of care within state guidelines. Productivity expectations noted by client.
