Minimum Requirements
- Associate's degree or higher in Physical Therapy or related field from an accredited program. Bachelor’s, Master’s, or Doctoral degrees accepted.
- Current Physical Therapy license in the state of Washington.
- Current CPR certification.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable public transportation.
- Ability to work independently in home settings across diverse patient populations and home environments.
- Experience providing skilled home health physical therapy, including evaluation, development of plan of care, and documentation of progress notes per agency policy.
- Comfort with travel between patient homes. Mileage reimbursement provided per client policy.
- Minimum education requirement: Associate Degree. (Client requires professional certification and state licensure as listed.)
- Preferred: Team-player attitude and initiative.

- Weekly hours: 36 hour guarantee.
- Coverage area: Grant, Adams, and Lincoln counties; candidate must travel between patient homes within these counties.
- Productivity expectations: 30 points (client metric).

Additional Information
Evaluates patient functional status and physical therapy needs. Develops and follows PT plan of care consistent with state law and agency policy. Provides treatment to relieve pain, restore or maintain function, and educates patients and caregivers. Completes evaluations, progress notes, and documents outcomes per policy. Reports changes in condition to supervising nurse or physician.
