Minimum Requirements

• PT degree (Bachelor’s, Master’s, or Doctoral) from an accredited program approved by the APTA.
• Active Pennsylvania Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred. This role will function as the primary PT for the agency and requires completion of SOCs, evaluations, reassessments, discharges, establishment of POCs, supervision of a PTA, caseload management, and daily patient outreach and scheduling.
• Experience with electronic documentation systems desirable. Knowledge of HCHB and OASIS is preferred.
• Ability to travel within York County and western Adams County as part of regular patient visit coverage.
• Valid driver’s license and ability to document mileage between patient visits (mileage reimbursement provided per client policy).

Assignment Details

• Facility Type: Home Health.
• Shift: Days.
• Shift Hours: 8:00 AM – 4:00 PM (client indicates 8:00 AM – 4:30 PM for scheduling; schedule is Monday through Friday).
• Weekly Hours: Typically 32–40 hours per week; weekly guarantee noted by client is 32–40.
• Weekend/On-call Requirement: None.
• Primary responsibilities: serve as primary PT, perform SOCs, evaluations, reassessments, discharges, establish Plans of Care, supervise PTA, manage independent caseload, and perform daily patient outreach and scheduling.
