Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Texas Physical Therapy license required to start the assignment.
• Current BLS certification from AHA or ARC may be required.
• Home health experience required. Candidate must have experience with home health evaluations, start-of-care visits, 30-day reassessments, treatment visits, and discipline discharges.
• Experience with HCHB (HomeCare HomeBase) electronic medical record is required or strongly preferred.
• Ability to travel within coverage areas including Crockett, Grapeland, Elkhart, and Palestine during the assignment.
• Minimum education requirement: Bachelor's Degree as noted by the client.
• Professional Physical Therapy license required.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0800-1600 (8:00 AM - 4:00 PM) with schedule indicating 5x8.
• Weekly Hours Guarantee: 36 hours per week.
• On-call/Weekend Requirement: none.
• Coverage Area: Candidate should expect patient visits in Crockett, Grapeland, Elkhart, and Palestine; mileage between visits is reimbursed.
