Minimum Requirements

• Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Minimum education requirement: Master’s.
• Active Florida Physical Therapist license required prior to start of assignment.
• Home health experience required. Experience providing physical therapy in patients' homes is mandatory.
• Experience with HCHB (Homecare Homebase) and OASIS documentation required.
• BLS certification from AHA or ARC may be required.
• Experience working in orthopedic care and willingness to participate in orthopedic weekend rotations as required by the client.
• Independent contractor status; candidate will support Care Services, Clinical segment.
• Preferred: familiarity with LHC Group general requirements and LHC dress code.

Assignment Details

• Facility Type: Home Health.
• Shift: Days.
• Shift Hours: 08:00 - 17:00 (M-F 8:00a-5:30p per client schedule).
• Weekly Hours Guarantee: 40 hours.
• Coverage Areas: Osceola County and Orange County.
• Weekend Requirement: Orthopedic rotation on weekends.
• Mileage reimbursement: Yes, mileage reimbursed between visits from first patient to last patient.
