Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Illinois PTA license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross preferred.
• Experience working in a skilled nursing facility required.
• Experience providing restorative and geriatric therapy preferred.
• Ability to collaborate with interdisciplinary team and follow care plans.

• Facility type: Skilled Nursing Facility.
• Shift: Days.
• Shift hours: 08:30–16:30.
• Schedule: 5x8.
• Floating: Candidate may be required to float to other facilities as needed.
