Minimum Requirements

• Doctoral degree in Physical Therapy (DPT) from an accredited program.
• Active Washington state PT license required prior to start of assignment.
• BLS/CPR certification required (American Heart Association or equivalent).
• Home health experience required. Experience with SOC, evaluations, ROC, recertifications, and subsequent visits preferred.
• Familiarity with home health electronic medical records preferred. Previous use of HCHB EMR is a plus.
• Ability to travel within a 50 mile radius for patient visits.
• Business casual dress expected.

• Shift Details: Days 0800-1600.
• Weekly Hours: 40 hours per week guaranteed.
• On Call/Weekend Requirement: No.
• Coverage Area: Up to 50 mile radius from base location.
• Primary Responsibilities: Start of Care (SOC), evaluations, recertifications, return-to-care (ROC), and follow-up visits.
