Minimum Requirements
- Doctoral degree in Physical Therapy (DPT) from an accredited program.
- Active Washington state PT license required prior to start of assignment.
- BLS/CPR certification required (American Heart Association or equivalent).
- Home health experience required. Experience with SOC, evaluations, ROC, recertifications, and subsequent visits preferred.
- Familiarity with home health electronic medical records preferred. Previous use of HCHB EMR is a plus.
- Ability to travel within a 50 mile radius for patient visits.
- Business casual dress expected.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Days 0800-1600.
- Weekly Hours: 40 hours per week guaranteed.
- On Call/Weekend Requirement: No.
- Coverage Area: Up to 50 mile radius from base location.
- Primary Responsibilities: Start of Care (SOC), evaluations, recertifications, return-to-care (ROC), and follow-up visits.
