Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current, unencumbered South Carolina physical therapy license required prior to start of assignment.
• Home Health experience required. Must be familiar with home visit workflows and documentation.
• Experience supervising and evaluating Physical Therapy Assistants and Home Health Aides preferred.
• Ability to perform patient evaluations and provide therapeutic services per physicians' orders, including development and updating of plans of care.
• Reliable transportation, valid driver’s license, and agency-required liability insurance required for home visits.
• Current CPR certification required.
• Proficiency with basic computer functions, electronic documentation, email, and online training platforms preferred.
• 2–3 years of physical therapy experience preferred.

• Facility Type: Home Health.
• Shift Name: Evenings.
• Schedule Pattern: 5x8.
• Direct supervisory responsibilities: Supervises and evaluates PTAs and Home Health Aides as assigned.
• Must perform home visits and evaluate home environments as part of treatment and discharge planning.
