Minimum Requirements

• Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapy license required to start the assignment.
• Home health experience required. Experience with OASIS assessments and documentation using HCHB required.
• Experience performing admissions, recertifications, 30-day assessments, and discharges in a home health environment.
• Experience supervising PTAs or coordinating care with PT assistants required.
• BLS certification from AHA or ARC may be required.
• Minimum education requirement: Associate degree (client requirement).
• Preferred: prior travel or remote caseload management experience. Preferred: familiarity with mileage reimbursement processes between visits.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Schedule: Monday through Friday, typical daytime hours (client indicates Mon–Fri am to 4:30 pm).
• Weekly guaranteed hours: 32 hours per week.
• Productivity expectation: 30 points.
• Coverage area: Logan and Todd counties. Mileage reimbursed between first and last patient visits.
• On-call/weekend requirement: No.
• Dress code: royal blue scrubs.
• Floating: Candidate may be expected to cover patients across the specified coverage areas.
