Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Wisconsin PT license required to start the assignment.
- BLS (Basic Life Support) certification from AHA or American Red Cross may be required.
- Experience working in skilled nursing facility (SNF) settings preferred.
- Demonstrated experience with restorative nursing, falls prevention programs, and Medicare documentation preferred.
- Ability to manage caseload independently and collaborate with interdisciplinary teams.

- Shift Details: 5 days per week.
- Start: Immediate start available.
- Floating: Candidate may be required to float to other facilities as needed.
