Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Current Physical Therapy licensure in Washington state or a compact license with privilege to practice in Washington.
- Current CPR certification.
- Valid driver’s license, vehicle insurance, and reliable vehicle for patient visits.
- Experience providing home health physical therapy, including patient evaluations, development of plan of care, and CMS documentation.
- Ability to complete progress notes per agency policy and prepare discharge summaries at required intervals.
- Willingness to travel throughout Pierce County for patient visits and to cover other nearby agency service areas as needed.
- Must meet client-specific expectations: ability to meet productivity targets (30 points) and a weekly hour guarantee of 36 hours.
- Minimum education requirement: Bachelor’s Degree.
- Preferred: experience with HomeCare HomeBase electronic documentation.

- Shift details: Monday to Friday, 08:00 AM – 04:00 PM.
- Weekly guaranteed hours: 36.
- Productivity expectation: 30 points.
- Coverage area: Pierce County, WA.
- Mileage reimbursed between visits from first patient to last patient.
- No on-call or weekend requirement per client; schedule flexibility and occasional weekend or holiday availability may be needed to meet patient needs.
- Candidates may be expected to float to other nearby agency service areas as needed.
