Minimum Requirements

• Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky PT license or ability to obtain state licensure prior to start.
• Current CPR/BLS certification.
• Experience with Home Health setting, including start-of-care evaluations, recertifications, and discharges.
• Familiarity with Home Health documentation systems such as HCHB or PointClickCare/Pointcare.
• Ability to travel within coverage areas: Boyle, Garrard, and Madison counties.
• Willingness to work assigned shift schedule (see Assignment Details).
• Minimum education requirement: Associate degree.
• Preferred: Prior home health travel experience and familiarity with mileage reimbursement processes between visits.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Evenings.
• Shift Details: Evenings 2000-0400.
• Days: 5x8 schedule.
• Coverage Areas: Boyle, Garrard, and Madison counties.
• Productivity Expectations: 30 points (client-specific).
• Weekly Hour Guarantee: 40 hours.
• On-call/Weekend Requirement: No call requirement.
• Mileage reimbursed between visits from first patient to last patient.
• Required Certifications: PT License and CPR.
• Dress Code: Gray, Black, Blue scrubs or business casual.
