Minimum Requirements

• Associate's, Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapist license required to start the assignment.
• BLS certification required.
• Experience with OASIS documentation.
• Experience with HCHB or PointClickCare (PointCare) electronic documentation.
• Home health experience preferred.
• Minimum education requirement: Associate degree (client-specific).

Assignment Details

• Primary specialty: Physical Therapist.
• Facility type: Home Health.
• Shift: Days.
• Shift hours: Monday–Friday 0800–1600 (client notes indicate Monday–Friday 0800–1630 EST schedule and Days 0800–1600).
• Weekend/on-call requirements: One weekend per month and one weekday on-call per week.
• Coverage areas: Madison, Montgomery, Clark, Powell counties.
• Typical responsibilities: SOC, evaluations, recertifications, ROC, agency discharges within scope of practice.
• Mileage reimbursed between visits from first patient to last patient.
• Floating to other facilities is not indicated. Follow facility assignment as directed.
