Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Michigan Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home Health experience preferred; open to candidates new to the home health setting.
- Experience with Home Care Home Base EMR preferred.
- Productivity expectation: ability to meet approximately 30 points per week.
- Willingness to travel within approximately a one-hour radius of the hub city to cover patient referrals.
- Interview via Teams or video call.

- Shift hours per week: 36.
- Weekend rotation: approximately one weekend per month, as needed.
- Mileage reimbursement: IRS standard mileage rate.
- On call: None.
