Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Michigan Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home Health experience preferred; open to candidates new to the home health setting.
• Experience with Home Care Home Base EMR preferred.
• Productivity expectation: ability to meet approximately 30 points per week.
• Willingness to travel within approximately a one-hour radius of the hub city to cover patient referrals.
• Interview via Teams or video call.

• Shift: Days.
• Shift hours per week: 36.
• Weekend rotation: approximately one weekend per month, as needed.
• Mileage reimbursement: IRS standard mileage rate.
• On call: None.
