Make a meaningful difference as a travel Occupational Therapist. We are seeking a dedicated and compassionate Occupational Therapist to help patients with injuries, illnesses, or disabilities regain or improve daily living skills while working in home health settings in Wallingford, CT.

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Occupational Therapy from an accredited program.
• Active Connecticut state OT license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience is required. Experience providing in-home evaluations, home safety assessments, and ADL/IADL training preferred.
• Minimum education requirement: Bachelor's Degree as specified by the client.
• Ability to provide services across multiple coverage areas within the region as needed.
• Professional certification required as noted by the client. Candidates must maintain required professional credentials and documentation.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Shift schedule: Monday through Friday.
• Weekly hour guarantee: 32 hours per week.
• On call/weekend requirement: No.
• Mileage reimbursed between patient visits from first patient to last patient.
• Coverage areas include multiple towns surrounding Milford, CT and Waterbury, CT; candidate must be able to travel within assigned coverage region.

Additional Role Information

Top responsibilities mirror standard LHC home health OT duties, including conducting in-home assessments, developing and implementing individualized treatment plans, performing home safety and adaptive equipment recommendations, documenting clinical progress, and coordinating care with interdisciplinary teams. Dress code is business casual or scrubs. Candidates should be prepared to follow agency policies and documentation standards.
