Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active New York State Physical Therapist Assistant license required prior to the start of the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility is required.
• Experience with transfers, gait training, therapeutic exercise, and mobility devices (e.g., walkers, canes) preferred.
• Demonstrated ability to document skilled services per facility and payer requirements.
• Reliable clinical communication skills for collaboration with interdisciplinary teams.
