Minimum Requirements
- Doctoral Degree in Physical Therapy (DPT) from an accredited program.
- Active Washington State PT license is required to start the assignment.
- CPR certification required.
- Home health experience required. Experience conducting SOC, evaluations, ROC, recertifications, and subsequent visits.
- Experience with HCHB EMR preferred.
- Ability to cover patients within a 50 mile radius from base location.
- Proof of eligibility to work in the U.S.
Assignment Details
- Facility Type: Home Health.
- Shift: Days.
- Shift Hours: 08:00 - 16:00 (days).
- Schedule: 5x8, 40 hours per week guaranteed.
- On-call/Weekend Requirement: No.
- Mileage reimbursed between patient visits from first patient to last patient.
- Primary responsibilities: SOC, evaluations, return-of-care visits, recertifications, and subsequent visits.
- Dress code: Business casual.
- May be required to float within the local coverage area as needed.
