Minimum Requirements
- Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Oregon Physical Therapist license required to start the assignment.
- Current BLS (Basic Life Support) certification from AHA or ARC preferred. If required by client, BLS must be valid prior to start.
- Experience providing home health physical therapy services. Client prefers clinicians with prior home health caseload management, documentation experience, and ability to work independently in community settings.
- Valid driver’s license and reliable transportation for travel within Jackson County. Client requires clinicians to travel to patient homes across the coverage area.
- Ability to perform patient assessments, develop skilled treatment plans, and provide skilled interventions for adult and geriatric populations. Prior experience with mobility training, fall prevention, and ADL retraining preferred.
- Demonstrated proficiency with electronic medical records documentation. Client may require timely completion of visit notes and communication with interdisciplinary team.
