Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Physical Therapist license in Indiana required prior to the start of assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred. (Preferred)
• Experience conducting SOC (start of care), evaluations, and treatment of physical therapy eligible patients in a home health environment.
• Familiarity with home health documentation systems such as HCHB or PointCare is preferred. (Preferred)
• Ability to travel within and provide visits across service areas including Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley counties.
• Minimum education: Bachelor's Degree.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Hours: 08:00–16:00 (Day shift).
• Weekly Hours Guarantee: 36 hours per week.
• Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley Counties.
• Mileage reimbursement provided between visits, from first patient to last patient.
• On-call/weekend requirement: None.
• Primary responsibilities: SOC, evaluation, and treatment of PT-eligible patients in the home health setting.
• Required license: Licensed Physical Therapist in Indiana.
