Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Physical Therapist license in Indiana required prior to the start of assignment.
- BLS certification from AHA or ARC may be required.
- Home health experience preferred. (Preferred)
- Experience conducting SOC (start of care), evaluations, and treatment of physical therapy eligible patients in a home health environment.
- Familiarity with home health documentation systems such as HCHB or PointCare is preferred. (Preferred)
- Ability to travel within and provide visits across service areas including Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley counties.
- Minimum education: Bachelor's Degree.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Hours: 08:00–16:00 (Day shift).
- Weekly Hours Guarantee: 36 hours per week.
- Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells and Whitley Counties.
- Mileage reimbursement provided between visits, from first patient to last patient.
- On-call/weekend requirement: None.
- Primary responsibilities: SOC, evaluation, and treatment of PT-eligible patients in the home health setting.
- Required license: Licensed Physical Therapist in Indiana.
