Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active NC State Physical Therapy License required to start the assignment.
  • BLS certification required from AHA or ARC.
  • Minimum of 1 year of Home Health physical therapy experience. First-time travelers will be considered if experienced in home health (preferred).
  • Proficiency with HCHB (Homecare Homebase) documentation workflow.
  • Experience with home health tasks including evaluation, reassessment, discharges, and supervision of PTAs.
  • Valid driver’s license and proof of auto insurance. Ability to drive long daily routes with average mileage of 60–80 miles per day; mileage reimbursed weekly.
  • Comfort using mobile documentation tools such as EPIC Rover (view), Focura uploads, and company-issued smartphone (Samsung Note or similar).


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Hours: 08:00–17:00.
  • Schedule: 5x8, standard 40 hours per week; typically 6 patient visits per day with a requirement of 30 visit equivalents per week.
  • Duration: 13-week travel assignment.
  • Orientation: New employee orientation plus 5 days of clinical orientation.
  • On-call / Holidays: No call. No major holiday requirement. Weekend rotation: none.
  • Travel/Driving Expectations: Average daily drive 60–80 miles. No company vehicle provided. Company phone provided for documentation and patient/provider contact.
  • Floating: Candidate may travel throughout a multi-site catchment area including surrounding towns and communities; floating to other locations within the regional service area is required.

Lake Norman lies about 20–30 miles north, providing boating, fishing, and waterfront parks along a 5200-acre lake. SouthPark and the Epicenter in Uptown provide shopping districts and entertainment within a 10–25 minute commute. The U.S. National Whitewater Center is roughly 12 miles from downtown and offers rafting, climbing, and mountain biking for an adventurous lifestyle. Charlotte Douglas International Airport is about a 15–20 minute drive from central Uptown for regional and national travel. The area combines an urban professional environment with suburban neighborhoods and accessible greenways for running and cycling.
