Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Illinois PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility preferred.
• Experience with documentation in electronic medical records preferred.
• Ability to perform therapeutic interventions under PT supervision, including gait training, transfers, and therapeutic exercises.
• Client requires 5x8 day shift coverage; candidate must be available for the scheduled days and hours.
