Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active California PTA license required to start the assignment.
• BLS certification from AHA or ARC required prior to start.
• Minimum of 1 year of experience working in a skilled nursing facility, long-term care, or post-acute rehabilitation setting preferred.
• Experience providing restorative and skilled physical therapy services to geriatric patients preferred.
• Familiarity with documentation in electronic medical record systems and Medicare/Medicaid skilled nursing documentation requirements preferred.
• Ability to collaborate with interdisciplinary teams and follow plan of care set by supervising PT.

• Shift Name: Days.
• Shift Details: 5 days per week.
• Start timeframe: mid June start window; facility requests two clinicians (two PTAs, two PTs, or one of each).
