Minimum Requirements
- Bachelor's, Master's or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Arizona PT license is required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Outpatient orthopedic experience required. Must be able to perform comprehensive evaluations, develop and implement treatment plans, and document per clinic and payer requirements.
- Experience with outpatient documentation systems including letters of medical necessity, goal writing, and charge entry preferred.
- Ability to supervise and delegate to therapy assistants and to participate in student clinical education is preferred.
- Maintain professional licensure and complete state-mandated continuing education as required.

- Facility Type: Outpatient clinic.
- Shift Pattern: 5x8 schedule.
- Assignment Duration: 13 week assignment.
- Caseload Focus: Orthopedic outpatient physical therapy evaluations and treatment.
- Responsibilities include comprehensive evaluations, individualized treatment planning, documentation, and participation in performance improvement initiatives.
