Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• Active Kentucky Physical Therapist license is required to start the assignment.
• BLS Certification from AHA or ARC may be required.
• Experience in outpatient neuro or inpatient rehab settings preferred. Clinical rotation in outpatient neuro acceptable as preferred experience.
• Experience completing comprehensive evaluations, developing treatment plans and discharge planning within established lengths of stay.
• Demonstrated competency with documentation including daily notes, goals, functional quality indicators, letters of medical necessity, and discharge summaries in an electronic health record.
• Ability to collaborate with interdisciplinary teams, perform equipment trialing/ordering, and provide patient and family training including home exercise programs.
• Willingness to participate in student supervision and professional development activities.
• Compliance with state Practice Act, professional licensure in good standing, and required continuing education.

• Shift: Day (5x8).
• Setting: Outpatient neuro setting within an inpatient rehab-focused organization.
• Duration: 13 week assignment.
