Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Indiana Physical Therapy license required to start the assignment.
- BLS (Basic Life Support) certification from AHA or ARC may be required.
- Outpatient physical therapy experience required. Candidate should have experience evaluating patients, developing and implementing individualized treatment plans, and performing documentation including goal writing and letters of medical necessity.
- Experience with functional treatment programs and safe patient handling techniques preferred.
- Ability to supervise or provide direction to support staff and students preferred.
- Maintain professional licensure and meet state-mandated continuing education requirements. Adhere to discipline-specific state practice act and professional codes of conduct.
