Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active Pennsylvania Physical Therapy license required to start the assignment
• Home Health experience required
• OASIS assessment experience required
• Reliable vehicle and ability to travel within Clearfield and Centre Counties
• BLS Certification may be required from AHA or ARC
• Associates degree minimum education requirement as listed by client
• Preferred: HCHB (Homecare Homebase) electronic documentation experience

Assignment Details

• Facility Type: Home Health
• Shift Name: Days
• Shift Details: Monday – Friday 8:00 AM – 4:30 PM (days)
• Weekly Hours: 36 hours guaranteed
• Coverage Area: Clearfield County and Centre County travel between patient visits required
• Responsibilities: OASIS assessments, development of plan of care, in-home physical therapy treatment, supervision of physical therapy assistants, electronic charting
• Mileage Reimbursement: Yes. Client reimburses mileage between visits at $0.50/mile.
• Dress Code: Royal blue or black scrubs
