Minimum Requirements
- Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Michigan Physical Therapy license required to start the assignment.
- BLS (Basic Life Support) certification from AHA or ARC may be required.
- Experience in skilled nursing or long-term care settings preferred.
- Experience providing restorative nursing therapy and skilled PT services, including gait training, therapeutic exercises, transfer training, and use of standard SNF equipment preferred.
- Ability to complete documentation according to facility and regulatory standards.
Assignment Details
- Facility Type: Skilled Nursing Facility.
- Shift Name: Days.
- Shift Details: Days 08:30–16:30.
- Typical workweek: 5 days per week, 8-hour shifts.
- Assignment Length: 13 weeks.
- Guaranteed hours: 35 hours per week.
- Candidates must be able to work in a long-term care environment and may be expected to coordinate care with interdisciplinary teams.
