Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Illinois PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in skilled nursing facility settings preferred.
• Experience with adult and geriatric populations preferred.
• Ability to follow documented treatment plans and collaborate with interdisciplinary teams.
• Client-specific requirement: must be able to split assignment between two nearby sites and travel between them as needed.
• Minimum 1 year of clinical PTA experience preferred.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Days 08:30–16:30.
• Work Schedule: 5 days per week, 8-hour shifts.
• Assignment Length: 13 weeks.
• Guaranteed Hours: 35 hours per week.
• Candidate may be required to split time between two neighboring sites and float between those sites.
