Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona Physical Therapist license required prior to the start of the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience working in skilled nursing facility settings preferred.
• Experience with geriatric rehabilitation, restorative programs, and mobility training preferred.
• Ability to complete documentation electronically using facility charting systems.
• Client requires a 13-week contract. Floating to other nearby facilities may be required.

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Monday through Friday, 08:30–16:30.
• Meal Break: 30 minutes.
• Contract Weeks: 13 weeks.
