Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Michigan Physical Therapist license required to start the assignment.
• BLS Certification from AHA or ARC may be required.
• Experience with Home Health setting preferred. Candidates new to home health will be considered.
• Familiarity with HomeCare HomeBase EMR preferred.
• Ability to meet productivity expectations of approximately 30 points per week.
• Reliable transportation and willingness to travel within the coverage area, generally Ann Arbor/Chelsea/Dexter and surrounding communities within about a one-hour radius.
• Valid driver's license and ability to use personal vehicle for patient visits; mileage reimbursed at IRS standard mileage rate.
• Interview via Teams or video call required.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Days: Monday through Friday, with one weekend per month rotation.
• Coverage Area: Ann Arbor, Chelsea, Dexter, MI and surrounding communities within approximately a one-hour radius depending on referrals.
• EMR: HomeCare HomeBase.
• On Call: None.
• Weekend Rotation: One weekend per month; may not be necessary every month.

Ann Arbor features an urban downtown with art galleries, the Ann Arbor Art Fair (major annual event), and a well-regarded restaurant scene along Main Street and Kerrytown within a short commute. For weekend getaways, drive roughly 40 minutes to Lake Erie shoreline areas or about 35–50 minutes to larger parks and natural areas, fitting both suburban and active lifestyles.
