Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Michigan Physical Therapist license required to start the assignment.
  • BLS Certification from AHA or ARC may be required.
  • Experience with Home Health setting preferred. Candidates new to home health will be considered.
  • Familiarity with HomeCare HomeBase EMR preferred.
  • Ability to meet productivity expectations of approximately 30 points per week.
  • Reliable transportation and willingness to travel within the coverage area, generally Ann Arbor/Chelsea/Dexter and surrounding communities within about a one-hour radius.
  • Valid driver's license and ability to use personal vehicle for patient visits; mileage reimbursed at IRS standard mileage rate.
  • Interview via Teams or video call required.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Days: Monday through Friday, with one weekend per month rotation.
  • Coverage Area: Ann Arbor, Chelsea, Dexter, MI and surrounding communities within approximately a one-hour radius depending on referrals.
  • EMR: HomeCare HomeBase.
  • On Call: None.
  • Weekend Rotation: One weekend per month; may not be necessary every month.

For weekend getaways, drive roughly 40 minutes to Lake Erie shoreline areas or about 35–50 minutes to larger parks and natural areas, fitting both suburban and active lifestyles.
