Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Colorado Physical Therapist license is required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience working in a skilled nursing facility or long-term care setting preferred.
• Proficiency evaluating and treating patients with common SNF diagnoses, including post-operative care, stroke, orthopedic conditions, and deconditioning.
• Experience with documentation in electronic medical records systems preferred.
• Demonstrated ability to work with interdisciplinary teams and develop individualized care plans.
