Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• Active California Physical Therapy license required to start the assignment.
• BLS Certification from AHA or ARC may be required.
• Experience in Skilled Nursing Facility (SNF) or Acute Rehab settings is required; SNF experience preferred.
• Experience providing primarily 1:1 patient care; minimal concurrent or group treatment preferred.
• Familiarity with NetHealth and PCC EMR systems preferred.
• Ability to perform productivity expectations: approximately 85% production; evaluations commonly 90 minutes, follow-up visits 15–45 minutes.
• Comfort working in a short-term, 45-bed SNF focused on short-term/acute care (no long-term, ALF, or memory care).
• Resume required for submission. Preferred: prior experience in fast-paced short-term SNF or acute rehab environments.

• Facility Type: Skilled Nursing Facility (45-bed, short-term focus).
• Shift Name: Days.
• Schedule: Monday through Friday.
• Patient Care Model: 1:1 care preferred; try to avoid concurrent and group treatments.
• EMR Systems: NetHealth and PCC.
