Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Colorado Physical Therapy license or ability to obtain license prior to start of assignment.
- BLS certification from AHA or ARC may be required prior to start.
- Must be a licensed Physical Therapist (PT only). PT Assistants (PTAs) will not be accepted for this role.
- Outpatient clinic experience, including performing 60-minute initial evaluations and 30-minute follow-up treatments.
- Experience treating a broad adult outpatient population and developing individualized treatment plans focused on functional return to community activities.
- Documentation competency including evaluation notes, goal writing, letters of medical necessity, quality indicator assessment, and timely charge entry.
- Ability to supervise and delegate therapy services as appropriate and participate in student clinical education when assigned.
- Demonstrated adherence to professional codes of conduct and the State Practice Act; maintain required continuing education and licensure in good standing.
- Preferred: Prior experience working in multi-site outpatient settings or split-site schedules.
Assignment Details
- Facility Type: Outpatient clinic.
- Schedule: 40 hours per week, 5x8 format.
- Evaluations: 60-minute initial evaluations; 30-minute follow-up visits.
- Patient Population: General outpatient adult caseload.
- Floating: Position requires splitting coverage between two clinic locations within the same metro area.
- Contract Duration: 13-week assignment with client preference for a 6-month engagement.
