Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Colorado Physical Therapist license required to start the assignment.
• Current BLS certification from AHA or ARC may be required.
• Experience in outpatient physical therapy settings required. Must be comfortable providing 60-minute evaluations and combined eval/treatment sessions.
• Demonstrated competency with documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry per clinic policies.
• Ability to develop and modify individualized treatment plans and to delegate and direct discipline-specific therapy services as needed.
• Preferred: Experience working across split-site or multi-location outpatient clinics and familiarity with outpatient EMR systems.

• Facility Type: Outpatient clinic.
• Shift Pattern: 5x8 schedule.
• Patient Care Model: 60-minute evaluations plus treatment sessions.
• Floating Requirement: Candidate may split time between two clinic locations within the region.
• Assignment Duration: 13 week assignment.
