Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Texas Physical Therapist license required to start the assignment.
  • Master's degree minimum educational requirement as listed by the client.
  • Current BLS certification from AHA or ARC may be required.
  • Home health experience. Client prefers at least 2 years of home health experience.
  • Experience with HCHB electronic documentation system required.
  • Experience supervising PTAs is required.
  • Ability to meet productivity expectations: approximately 30 points per week, including OASIS and treatment visits.
  • Documentation completed within 24 hours of visits.
  • Ability to attend weekly case conference on Thursdays at 9:00 AM.
  • Valid driver's license and ability to travel between patient visits. Mileage reimbursement provided between visits from first patient to last patient.
  • Preferred: experience in high-volume agencies and working with high-acuity patients.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Evenings.
  • Shift Hours: Evenings 2000-0400.
  • Weekly Hour Guarantee: 32 hours per week.
  • Productivity Expectation: 30 points per week.
  • Coverage Area: Dallas service area. Travel between patient homes required.
  • Weekend Requirement: One weekend per month on call/coverage.
  • Clinical Duties: Responsible for OASIS assessments, treatment visits, and supervision of PTAs.
