Join a home health team in Farmington Hills, MI as a traveling Physical Therapist. Minimum Qualifications
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Active Michigan Physical Therapy license required prior to start.
- 1-2 years of home health physical therapy experience preferred and/or required by client.
- Experience with HCHB electronic medical record system.
- Proficiency with OASIS and home health documentation and regulatory requirements.
- BLS certification required.
- Demonstrated ability to work independently in the field and manage time, territory, and productivity expectations.
Preferred Qualifications
- Strong clinical assessment, critical thinking, and decision-making skills in home health populations.
- Experience coordinating care with physicians, nursing, OT, ST, aides, and case managers.
- Experience recommending assistive devices and home modifications to improve safety and independence.
- Effective communication skills for patient, family, and interdisciplinary interactions.
Assignment Details
- Shift Schedule: Monday-Friday, 8:00 AM–4:30 PM.
- Weekly guarantee: 32 hours.
- Primary coverage areas include Wayne and Oakland counties, with patients also located across Macomb, Washtenaw, and Livingston counties. Clinicians are expected to travel where patient needs require.
- Mileage reimbursement between visits at $0.49 per mile.
- Travel from home to home for visits and manage geographic territories and visit cadence.

Impacting Patient Care
Provide thorough evaluations of strength, mobility, balance, pain, and safety risks. Develop individualized plans of care with measurable functional goals and implement skilled interventions including therapeutic exercise, gait and transfer training, fall prevention, and pain management. Educate patients and caregivers on home exercise programs and safe mobility techniques. Monitor progress, perform reassessments, and document clinical necessity and outcomes in compliance with Medicare and regulatory standards. Collaborate with interdisciplinary team members to coordinate services and support transitions of care.
