Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active New York State Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required prior to or upon start.
- Experience working in skilled nursing, long-term care, or rehabilitation settings preferred.
- Demonstrated ability to evaluate, develop, and implement individualized treatment plans for geriatric and post-acute patients preferred.
Preferred Qualifications
- Prior travel therapy experience preferred.
- Proficiency with electronic medical records preferred.
- Certifications in specialty areas such as Geriatric Certified Specialist or NF-1 preferred.
Assignment Details
- Shift hours: 08:30 - 16:30.
- Work pattern: 5 shifts per week.

Impacting Patient Care
Deliver skilled physical therapy services in a long-term care environment. Provide evaluations, therapeutic interventions, discharge planning, and caregiver education to improve patient function and quality of life. Collaborate with interdisciplinary teams to support resident-centered care and compliance with facility protocols.
