Minimum Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in home health settings, including assessing and evaluating therapeutic/rehabilitative/functional status and developing the plan of care.
• Experience completing comprehensive OASIS assessments when appropriate.
• Experience evaluating home environments and documenting care using electronic systems; familiarity with Kinser preferred.
• Ability to manage a caseload of at least six patients per day.
• Current resume required to submit for consideration.

Preferred Qualifications

• Prior home health travel experience.
• Experience with varying patient populations and complex discharge planning.

Assignment Details

• Home health caseload requiring a minimum of six patients per day.
• Documentation completed in Kinser.

Impacting Patient Care

Assess and evaluate patients’ functional and rehabilitative status in their homes. Contribute to individualized plans of care and perform comprehensive OASIS assessments when appropriate. Provide skilled therapeutic intervention and collaborate with interdisciplinary teams to support patient independence and safe home functioning.
