Minimum Qualifications
- Current, active Physical Therapist (PT) license required for Indiana prior to assignment start.
- Home health experience required.
- Experience with Resumption of Care, Start of Care, and Discharges in home health settings.
- Experience supervising or providing oversight to PTAs.
- Familiarity with electronic medical record systems; experience with HCHB required.
- eMAR system experience required.
- Minimum education: Associate degree in a related field required; Bachelor’s, Master’s, or Doctoral degree in Physical Therapy is acceptable where applicable.
- BLS certification may be required from AHA or ARC.
Preferred Qualifications
- Strong communication skills preferred.
- Prior experience covering Marion and Hamilton county areas preferred.
Assignment Details
- Shift hours: 0800 - 1600 (M–F schedule reflected in order notes as M–F 0800–1630).
- Weekly hour guarantee: 36.
- Productivity expectations: 30 points.
- Mileage reimbursement between first and last patient visit: Yes.
- Coverage areas include Marion and Hamilton counties. Candidates may be required to travel within these coverage areas.
- No on-call or weekend requirement.

Impacting Patient Care
Provide skilled physical therapy in patients’ homes, focusing on recovery, functional improvement, and safe discharge planning. Contribute to continuity of care through documentation, coordination with interdisciplinary teams, and oversight of PTA services. Support quality outcomes in a community-based care model.
