Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in Watsonville, CA providing home health services where you will help patients recover from injuries and manage chronic conditions while experiencing life in a new location.
Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active California Physical Therapy license required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Home health experience preferred.
- Productivity expectation: 30 visits per week.
Preferred Qualifications
- Experience with documentation in electronic medical records for home health.
- Previous travel or per diem home health experience preferred.
Assignment Details
- Shift schedule: 5 days per week.
- Role is in a home health setting; candidate may be expected to travel locally between patient homes.

Provide home health physical therapy that supports patient independence and recovery. Maintain continuity of care while working with interdisciplinary teams to meet patient goals. Document progress and communicate effectively with clinicians and families to support safe transitions and optimal outcomes.
